Voters with no mailing address in the Commission on Election's Certified List of Overseas Voters

The registered voters in the attached list: please click here. (format: Last Name, initials of First and Middle Names) DO NOT HAVE A MAILING ADDRESS reflected in the Commission on Election’s (COMELEC) Certified List of Overseas Voters (CLOV) for the 2022 National Elections. The Official Ballot cannot be sent unless a mailing address is provided.

In order to receive the Official Ballots, voters on the attached list are instructed to SEND BY EMAIL to chicagopcg@att.net (using the Subject: “CLOV Address”) a scanned copy of the following documents:

1. 1. Accomplished and signed OVF-1 form (click here to download the form)
2. 2. Valid Philippine passport; and,
3. 3. . Proof of current address (e.g., State ID, Driver’s License or Utility Bill).

Last day for submission of the above is on 06 April 2022.
