  REQUIREMENTS FOR ACCREDITATION

 

       Only the applications from organizations located within the following states under the Consulate General’s jurisdiction will be processed:

Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota,  Missouri, North Dakota, Nebraska, South Dakota, Ohio and Wisconsin 

 

Requirements:

 

I.                    For Organizations that will be registering for the first time:

                  1.      List of Officers (please include 2x2 photos, mailing address, phone number/s and email address)

2.      List of Members

3.      Copy of Constitution and Bylaws

4.      Plans and programs for the year

5.      A self-addressed stamped envelope

 

II.                  For organizations that will be renewing its registration:

 

1.      List of Officers (please include 2x2 photos, mailing address, phone number/s and email address)

2.      List of Members

3.      Copy of Constitution and Bylaws (if amended or revised)

4.      Annual report of activities (with photographs)

5.      Plans and programs for the year

6.      A self-addressed stamped envelope

 

Applications should be on Letter-sized (8.5 x 11) paper and enclosed in a folder and mailed to:

Accreditation is valid for one year.
